Course Details

Chronic illness has profound effects on a person's physical, emotional, and mental well-being, making it difficult to carry on with daily routines and relationships. As part of a healthcare team, Community Paramedics will provide individuals impacted by a chronic health condition with the support knowledge, skills and resources they need to better manage on a day-to-day basis. In this course learners will be introduced to the complexities and intricacies of chronic disease management, including assessment, treatment modalities and complications. Learners will look at common barriers to health promotion, prevention and self-management of chronic diseases at the community and individual levels. The course also looks at the impact of mental health on chronic health conditions as well as principles of self-management of chronic conditions in the geriatric population.
