Procedure Statement

Scope

This procedure forms part of JIBC’s Protection of Privacy and Access to Information policy (the “Policy”) and provides examples of Personal Information specific to the Institute and as contemplated under the Act. Terms not otherwise defined in this procedure are as defined in the Policy.

Defining Personal Information

The following are examples of Personal Information:

  • names, home addresses, home telephone numbers and personal email addresses;
  • age or birth date
  • gender;
  • marital or family status;
  • identifying number, such as a student or employee number, personal education number, social insurance number, personal health care number, driver’s license number or services card number;
  • race, national origin, or ethnic origin;
  • religious or political beliefs or associations;
  • blood type;
  • educational history, including information maintained on any student file or stored in a student information system and/or learning management system, including applications, references and evaluations, admission, enrolment, academic advising, assignments, grades, transcripts, test or assignment scores, coop work, placements, career counseling, volunteer activities, finances and financial assistance, academic dishonesty, misconduct investigations and discipline and extra-curricular activities;
  • employment history, including information maintained on any employee file or stored in any administrative information system, including applications, CVs, references, recruitment and hiring, performance evaluations, promotion and salary review, work schedules and absences, compensation, pension and benefits, finances and financial awards, grievances, misconduct and investigations and discipline, career counseling, professional development and training, outside activities, employment separation, termination and retirement;
  • medical history, such as information maintained on any patient, student, employee, or disability care file describing medical conditions, diagnoses, treatment and procedures, prescribed drugs, psychological and psychiatric evaluations and occupational health and safety;
  • disability status, including information maintained on any patient, student, employee, or disability case file describing physical or cognitive assessments, abilities, treatment, and accommodation;
  • financial history, including banking, tuition, loans, grants, tax information, donors and donations, and credit card information;
  • criminal history, including criminal records checks;
  • images, such as those within a student or employee photo identification system;
  • anyone else’s recorded opinions about an individual, including performance evaluations and letters of reference;
  • an individual’s recorded personal views or opinions; and
  • name, address and phone number of a parent, guardian, spouse or next of kin.
